Athlete Health Card.

Taking into consideration:

1. The provisions:

a) Law 1558/1985 "On Government and Government Bodies", art. 27 "Establishment of General Secretariats", par. 2e, para. in "General Secretariat of Sports" (Government Gazette 137/A'/26.07.1985).

b) Of the p.d. 63/2005 "Codification of the Legislation for the Government and the governmental bodies" (F.E.K. 98/A'/22.04.2005), as applicable.

c) Of the p.d. 70/2015 "Reconstitution of the Ministries of Culture and Sports, Infrastructure, Transport and Networks [...] Development and Tourism" (Government Gazette 114/A'/22.09.2015).

d) Of the p.d. 4/2018 "Organization of the Ministry of Culture and Sports" (Government Gazette 7/A'/22.01.2018), as applicable.

e) Of p.d. 125/2016 "Appointment of Ministers, Deputy Ministers and Deputy Ministers" (Official Gazette 210/A'/05.11.2016).

f) Of p.d. 73/2015 "Appointment of the Vice-President of the Government, Ministers, Deputy Ministers and Deputy Ministers" (Government Gazette 116/A'/23.09.2015).

g) Of p.d. 121/2017 "Organization of the Ministry of Health" (Official Gazette 148/A'/9.10.2017).

h) The no. YPPOA/375614/9900/11.11.2016 of the decision of the Prime Minister and the Minister of Culture and Sports, on the delegation of responsibilities to the Deputy Minister of Culture and Sports Georgios Vassiliadis (Government Gazette 3672/B'/11.11.2016) as amended by no. YPPOA/GDOA/DYA/TDAD/97295/4796/ 434/325/28.02.2018 (Government Gazette 852/B/09.03.2018) decision of the same.

2. Paragraph 9 of Article 33 of Law 2725/1999 (A'/121), as it was replaced by Article 16 of Law 4479/2017 (Official Gazette 94/A'/29.06.2017 ).

3. The under no. prot. 19EKA/3955/15.06.2018 recommendation of the National Center for Sports Research (E.K.A.E).

4. The fact that the provisions of this decision do not cause an expense to the state budget, we decide:

We define the conditions and the certification process of athletes' health by recording it in the Athlete's Health Card, as follows:

Article 1

Athlete Health Card

1. Athletes' health certification is recorded in a health card, is mandatory and is a condition for their participation in training and matches.

2. The Athlete's Health Card is an independent document and is not linked to the Sports Identity Card. It is a personal document of the athlete and is in his possession.

3. Compulsorily submitted to the secretariat or to the competent match referee of any type of sporting event of a competitive nature, in accordance with the regulations for the conduct of matches of the National Sports Federations, as a basic condition for participation in the match.

Article 2

Content

1. The Athlete Health Card includes the following information:

a) National Sports Federation

b) Name, surname, patronymic of athlete

c) Year of birth

d) Sports

e) Photograph

f) Sportsmanship Card number

g) A.M.K.A.

h) Calendar visa with certificate - signature of a doctor for the certification of participation in the sport (preparation and competitions).

i) Certificate - signature that he has undergone the medical examinations of par. 2 of article 3 hereof.

Article 3

Version and Power

1. The Athlete Health Card is issued and renewed by the National Sports Federations. It is issued and renewed free of charge, without any financial burden on sports clubs and athletes, and is valid for one (1) year from its issuance.

2. It is issued only after the following mandatory examinations:

a) Clinical examination

b) Individual and family medical history

c) Electrocardiogram.

3. Depending on the findings of the above mandatory tests and if the doctors of par. 1 of article 4 deem, according to their scientific judgment, that further investigation is necessary, they may refer the athlete for further tests.

Article 4

Visa

1. The Athlete's Health Card is considered by doctors of Primary Health Care service units, Prefectural, Regional or University Hospitals, health military units or by doctors having any relationship with the State or with N.P.D.D., as well as by doctors of the private sector, possessing the cardiology specialty.

2. Physicians of other specialties are given the opportunity to attend post-training seminars, which will be organized by the E.K.A.E., in matters of pre-competition control of athletes.

3. For the organization of post-educational seminars, E.K.A.E. may contract with national and international medical scientific bodies and companies.

4. The cost of post-training seminars may be covered by a price that will be borne by the post-training students and/or by sponsorships.

Article 5

Final Provisions – Effective Date

1. Within a period of one (1) month from the entry into force of this, the National Sports Federations must ensure that the sports associations are informed of their strength and the Health Cards of their athletes are issued.

2. The validity of this decision starts from its publication in the Government Gazette.
